| Medical providers can use this service offering to find out what drugs BWC covers in its outpatient medication formulary. To conduct a search, authorized users must enter at least three characters of a specific drug name. Search results will include: Unless indicated in the Restriction column, all dosage forms and strengths of a listed drug are eligible for reimbursement per Ohio Administrative Code 4123-6-21 Physicians who would like to see a particular drug added to the formulary may submit the Formulary Medication Request Form (MEDCO-35) to the Pharmacy & Therapeutics Committee for consideration.
Note: BWC has allowed for non-formulary drugs to be covered for up to 180 days in cases of true medical necessity where there is no other drug available to treat the allowed condition. |
