Employer demographic information

This service offering allows employers to notify BWC of changes to the following information on their Ohio workers’ compensation policy.

Update primary physical Ohio location
You must update your primary address with a physical Ohio location not a P.O. Box. Note: If you do not have a physical Ohio location, please provide an out-of-state physical location. If you enter an inaccurate address, the system will reject it and keep your current primary address. Provide the appropriate suffix for your street name (e.g., Road, Ave., St., etc.). Also include directional information when applicable (e.g., Wooster Road NW or N. Main St.).

  • Street address can be no more than 40 characters; city can be no more than 30. You must complete both fields before submitting updates.
  • Enter the two-character state abbreviation and a five-digit ZIP code. You must enter this information unless you have a foreign address.

Update mailing address
To update your mailing address, you may enter either a physical location or P.O. Box. If you enter an inaccurate address, the system will reject it and keep your current primary address. Provide the appropriate suffix for your street name (e.g., Road, Ave., St., etc.). Also include directional information when applicable (e.g., Wooster Road NW or N. Main St.).

  • Street address can be no more than 40 characters; city can be no more than 30. You must complete both fields before submitting updates.
  • Enter the two-character state abbreviation and a five-digit ZIP code. You must enter this information unless you have a foreign address. BWC has based much of its formatting on codes and abbreviations used by the U.S. Postal Service. You may also update/add your phone number, fax number, contact name and contact phone number for your mailing location.

Update foreign address
BWC captures foreign addresses differently. The street name is the same as that for U.S. companies. Enter the other information under city such as Toronto ONT M8X 2X3 CANADA. You will not see address data in any other fields. So, to update a foreign address:

  1. Enter the address (up to 40 characters) under Street address;
  2. Enter the city, state/province, postal code and country name (up to 30 characters) under City.

You also can use this service offering to do the following tasks.

  • Notify BWC of your new e-mail address (up to 500 characters).
  • Send BWC your company's Web site address (up to 245 characters).
  • Send BWC your Doing business as (DBA) name (up to 40 characters). If you do not have a DBA name, you can leave this line blank.
  • Request cancellation of your elective coverage.
  • Request cancellation of your Ohio workers’ compensation coverage.
  • Add and/or cancel corporate officer.
  • View your company's authorized representative.
  • View your company's current managed care organization.

If you need to update your policy name and/or federal identification number or you are transferring a business from one immediate family member to another, you will need to complete the Notification of Policy Update (U-117). Your assigned workers’ compensation policy number and responsibility for premium will not change as a result of completing the U-117. BWC will not issue a new policy number

Do not use the U-117 for situations where an employer succeeds, in whole or part, another employer in the operation of a business. If you are a new/successor employer and do not have a BWC policy, you will need to complete an Application for Ohio Workers’ Compensation Coverage (U-3). If you already have a policy with BWC, you will need to complete a Notification of Business Acquisition/Merger or Purchase/Sale (U-118) to notify BWC.
