If you do not have a current physician of record for your claim, you can search for one with Certified provider look-up.
|
If you would like to change your physician of record you have three options:

Injured workers can complete the Notice to Change Physician of Record (C-23);

Call 1-800-OHIOBWC;

Send us an e-mail.

If the current treating provider is not displayed, please contact the managed care organization (MCO).
