DFSP Safety Action Plan (DFSP-5)

Introduction
Employers participating at the advanced level of the Drug-Free Safety Program must complete and submit a DFSP-5 outlining the action steps they intend to implement/accomplish during the remainder of the program year. Although employers at the basic level are not required to submit an action plan, they should consider the benefits of completing one to monitor progress and improvements to their safety processes.


Required information
List all the safety action steps you intend to implement to improve specific areas of your safety processes. Based these action steps on the results of the Safety Management Self-Assessment (SH-26). Provide as much detail as possible for each step, including:

  1. A detailed description of the actions/activities to be implemented;
  2. The person responsible for monitoring the progress and completion of each item;
  3. The expected date for completing each item;
  4. The expected improvement to your safety process.
Note: You're not required to list activities in every section. You may prioritize and choose which sections and which activities you want to accomplish first. Also, you don't have to complete everything in the first year. Evaluate both short-term and long-term improvements.
