Name, address, phone number, fax number and email address are available for the following parties:

• Injured worker;
• Employer;
• Group risk/claim representative;
• Claim management representative;
• Employer risk/claim representative;
• Legal representative;
• Managed care organization;
• Physician of record;
• Treating provider.
