• Authorization for Direct Deposit
• W-4 Tax Information
• IT4 Ohio State Tax Withholding

Municipal Income Tax Witholding Form
From The Hub, go to Divisions, Finance, Payroll, Forms to access this form.

Note: Use Tax Finder, to complete the Municipality Name/Tax Code and the School District/Tax Code.
The Municipal Income Tax Withholding Form must be submitted to the BWC payroll department via email attachment or interoffice mail.
